An admin who rotates a TACACS+ shared secret needs to update it in all three phase-specific tables for that server, and this counter reveals if the accounting table specifically was missed. For example, this counter climbing while the authentication and authorization tables for the same server show no such errors is a strong sign only the accounting secret is out of sync.
