What is cienaCesCfmGlobalLoopbackStatsRxLBMInvalidFirstTLVOffset?

This is the loopback-specific version of the first-TLV-offset integrity check seen for CCMs: it counts incoming LBMs whose pointer to where optional TLV data begins does not match the frame's actual structure. Because a loopback test is typically initiated deliberately by a known remote operator, this counter incrementing usually means that remote device's CFM stack has a frame-encoding bug specific to LBM construction, rather than a random transient issue. If a specific remote NMS or tester tool consistently triggers this counter every time it runs a loopback test against this device, that tool's LBM generation is what should be reported as buggy.
