This is the cumulative count of multicast packets transmitted out this interface. On a server-facing vNIC, an unexpectedly high multicast count often traces back to a clustering, service-discovery, or streaming-media application on that host, so this counter is an early flag for that kind of traffic pattern even before anyone checks the workload directly. A multicast count that dwarfs the unicast count on what's supposed to be a plain data vNIC would prompt an admin to check whether a clustering heartbeat protocol is running on that server.
