This Unsigned32 returns the cienaCesPmInstanceIndex of the single PM Instance that was auto-created for the object indexes given in this row, per its DESCRIPTION. Ciena's PM system auto-creates one PM instance per physical entity (a port, a circuit) rather than making you provision one by hand, so this object exists purely to hand you the numeric key you actually need to walk the other PM tables (cienaCesPmStatistics, cienaCesPmIntervalProfile, and so on), since those are indexed by cienaCesPmInstanceIndex, not by the port identifiers you started with. For example, if you know a DS3 port's slot/port indexes but not its PM instance number, reading this object first (say it returns 4137) tells you to use index 4137 when you next query cienaCesPmTdmDS3PortCurrBinEsl for that same port.
