This 64-bit counter tracks the total number of SCSI commands this specific authorized remote initiator has sent to this local target device or port, using a wide counter specifically so high-speed hosts do not wrap it between polls. A network admin watching this value can spot one particular initiator suddenly hammering a target with commands, which often means a backup job or a misbehaving HBA driver on that exact host. For example, a backup server's initiator entry climbing by roughly 50,000 commands during a nightly backup window is expected, but the same jump at 3pm on a Tuesday would be worth investigating.
